HOW DO WE USE THE COA QUESTIONNAIRES?

Stakeholder surveys are used by COA Peer Reviewers to learn how key stakeholders perceive your organisation and experience its services. They are an important source of evidence that peer reviewers use when determining the ratings for many standards.

Following is a list of COA’s Stakeholder Surveys:

Advisory Body/Council Survey
Community Survey
Consumer Survey (English and Spanish)
Contractee Survey
Foster Parent Survey (English and Spanish)
Governance Survey
Personnel Survey
Supervisors and Managers Survey
EAP Stakeholder Survey
EAP Affiliate Survey
Member Agency Survey
Youth Survey (for organisations that serve mandated youth)

COA is happy to introduce a new paperless electronic (telephonic and web-based) survey process. This process is a cost-effective, streamlined means to survey your stakeholders.

Your COA Coordinator will send you the custom URLs (web addresses) and telephone numbers for each survey. You will be asked to distribute these surveys to stakeholders 6 months prior to your scheduled site visit date. Complete instructions for distributing survey information via the electronic process can be found below. Hard copy surveys and sample cover letters to accompany the surveys must be requested from your COA Coordinator.
